HomeCall Vets Provider Agreement — Summary

As a HomeCall Vets provider, you agree to:

1. Independent Contractor Status. You are an independent contractor, not an employee of HomeCall Vets. You are responsible for your own taxes, insurance, and equipment.

2. Scope of Practice. You will only perform services within your training, credentials, and the scope defined in the HomeCall Vets Provider Protocol. All clinical decisions require DVM oversight.

3. Client Safety. You will follow all HomeCall Vets safety protocols, including the Provider Home Visit Safety Protocol, and immediately report any safety concerns to dispatch.

4. Background Check. You consent to a background check as part of the onboarding process.

5. Confidentiality. You will not disclose client information, visit details, or proprietary platform information to any third party.

6. Insurance. You are responsible for maintaining appropriate liability insurance for your scope of practice.

7. Platform Fees. HomeCall Vets retains a platform fee from each visit. Your payout percentage is defined in your individual provider agreement.
